LGAP (Leanne George Animal Physiotherapy) data protection policy (May 2018)
As part of the service we provide to you we will collect and hold certain personal information, such as name, phone number, email address and address, pets name and details as required. We only collect the essential information. The data is then kept in a digital file on a password protected device (smart phone/iPad/computer), or paper documents are stored in locked secure storage, if paper documents are transferred to electronic storage the paper documents will be destroyed.
The data is only used to ensure I can effectively communicate with you. I will not pass any data/information onto other professionals involved with your animal unless consent has been given by you. Only essential data will be used to identify you and your animal in communication with other professionals.
Data will be kept for 10 years and then destroyed by deleting the files electronically or shredding/burning paper data.

LGAP privacy policy (May 2018)
Your privacy is very important to us as LGAP and your information will only be used to provide you with the best possible service we can.
As part of our service to you we will collect certain information about you that could be considered personal (name, address, phone number, email address). We will record the details of your animal, our clinical findings and notes from each of our sessions and We will also ask and record the details of your veterinary surgeon with whom your animal is registered with. This will be stored electronically on password protected equipment or paper storage in secure storage. By law All of this information will be kept for 7 years then destroyed either by deleting the files electronically or shredding appear copies.
By law (veterinary surgeons act 1966) We must have consent from your vet to treat your animal, if at this time they require/request a report of my findings I will do this either by telephone conversation or email only. We will only pass your information onto any other professional involved with your animal once consent has been gained from you.
Should you with to access your information at any time or change, erase or restrict the data being kept please contact me. However please be aware that we are only able to change your contact details (personal information) and are not able to adjust the clinical notes relating to your animals treatment sessions with us.
